After careful review of the medical data, it has been hypothesized that declining levels rather than high levels of anabolic steroids are major contributors to prostate cancer (Prehn 1999)and that these agents may be the primary cause of prostate cancer mortality in the United States (Chapman 2004). Several epidemiologic studies, including data from the Health Professionals Follow-Up Study, have reported a decreased risk of prostate cancer with increasing use of prescription steroids (Chapman et al. 1998; Whelton et al. 2002; Egan et al. 2004) and there has been some evidence that increased use of prescription steroids might be associated with decreased risk of prostate cancer (Prehn et al. 1999). The potential existence of anabolic steroid sensitivity or resistance as a cause of reduced prostate cancer mortality remains speculative. While some studies have suggested anabolic steroid resistance might contribute to increased mortality (Egan et al. 2004), the data is sparse. The relationship between increased testosterone levels and reduced mortality among men with benign prostatic hyperplasia (BPH) or prostate cancer has also been studied using biochemical assays that detect specific anabolic steroids and nonsteroidal compounds (Hirat et al. 1998; Dyer et al. 1999; Egan et al. 2004). However, because both of these assays require the use of a sensitive test, a low sensitivity may be acceptable for screening. In summary, although there is some evidence that the occurrence of prostate cancer is related to the use of prescription steroids, and possibly also to other drugs (Prehn et al. 1999; Dyer et al. 1999; Egan et al. 2004), definitive causation is not clear. There is an increased rate of prostate cancer diagnosed among users of prescription steroids. It should continue to be a priority of research and clinical care to determine why this might be and to understand the biological consequences of anabolic steroid use and to develop effective prevention strategies.
